This training aims to introduce the fundamental principles and approaches for the systematic investigation of process safety incidents and root cause analysis. Concepts related to the analysis of incidents and the evaluation of actions aimed at preventing recurrence are covered. The training content has been prepared in consideration of API RP 754 and CCPS guidelines and is structured with a focus on process safety incidents.
Day 1 – Understanding Incidents and Analysis Methods

• Incident classification approach: Tier 1, 2, 3, 4, and near-miss incidents within the scope of API RP 754
• Incident development model: Barriers and error chain
• Investigation process: Planning, team structure, and role distribution
• Data collection and evidence management: Interviews, field inspections, and data verification
• Root Cause Analysis (RCA) Methods
Day 2 – Application, Actions, and Organizational Learning
• Practical incident analysis: RCA study based on a realistic process safety scenario and review of the results
• Addressing corrective actions
• Preparing effective and clear incident investigation reports
• Communication and presentation: Conveying findings to management and field teams
• Organizational learning: Lesson learning, dissemination, and prevention of recurrence
• Common mistakes: Superficial analysis and incorrect root cause identification
